SCOT-MED LLC. require Medical Director or Pharmacist-in-Charge authorization for any purchases of prescription drugs or medical devices, be on file including a copy of the appropriate and current state licensing or Training Entity release form. For controlled substances, a valid DEA registration is required as well as a copy of the State Controlled Substance License / B.N.D.D. Certification where required and will require verification of Medical Director information or certification prior to sales/shipment of these items.
